Project Summary

Integrated Care [IC] is considered as central to adapting care provision processes to addressing the needs of especially chronically ill patients. In Germany, although legal conditions have enabled IC for at least 12 years, its diffusion rate is still low. Already known barriers to IC are typically characterized by interdependencies that cut across the individual, organizational or societal level. Towards this end, we have established an interdisciplinary research group and have developed an iterative research design that integrates diverse and yet interrelated sub-projects.
